Dr. Smith welcomes new patients into his care for gentle, effective diagnosis and treatment. Please call to make an appointment from 8 a.m.-5 p.m. Monday through Friday. Most insurance is accepted and flexible payment plans are available. Please call for details.
At our office, we respect your time and have a goal to keep your wait time under 10 minutes. To help us meet that goal, we kindly ask that you either download, print out, and complete the New Patient Form (PDF), or complete and submit the Online New Patient Form prior to your appointment.
For your first appointment, please be sure to bring your:
- Completed New Patient Form (PDF) (unless you submitted the Online New Patient Form)
- Insurance Cards
- Co-Payment Information
- List of Current Medications
- Referral Forms (if required)
- X-ray Films (if available)
